Experienced Nurse Practitioners are sought for per diem home health risk assessments throughout Strafford County, NH. These visits focus on Medicare Advantage plan members and take place in a variety of home environments, including locations such as Dover, Rochester, and Somersworth. Flexible scheduling allows you to choose shiftsMonday through Sundaythat fit your lifestyle, with a minimum weekly or bi-weekly hourly commitment required.

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan members
• Conduct thorough reviews of prior and current medical history, ongoing medical conditions, and medication usage
• Educate patients and their families on disease processes, medications, and wellness opportunities
• Identify gaps in care and present actionable recommendations for improvement
• Complete required documentation after each assessment
• There is no need to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in NH
• Board Certification through AANP or ANCC
• MSN from an accredited school of nursing
• Current CPR certification (BLS)
• At least 6 months of recent experience as a Nurse Practitioner preferred
