Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual and post-discharge in-home health risk assessments, each averaging about 45 minutes
- Evaluate patients medical history, current conditions, and medication usage
- Educate patients on their diagnoses and prescribed medications
- Identify care gaps and offer recommendations to help close them
- Function as a valued resource; no need to create treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in GA
- Graduation from an accredited MSN program
- Board Certification (AANP or ANCC)
- Current CPR/BLS certification
- Preferably at least six months of recent Nurse Practitioner experience, ideally in home health
