Step into a meaningful per diem opportunity making a difference in the lives of Medicare Advantage members. In this role, you will conduct home health risk assessments throughout Dent County, MOincluding Salem and Bunkersupporting individuals with annual evaluations and post-discharge visits. Each visit averages around 45 minutes, allowing you to provide high-quality care while maintaining flexibility.
Schedule:
- Flexible shifts available seven days a week
- Set your own schedule according to your availability
- Choose a weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage plan members
- Review medical history, assess current medical conditions, and document medication history
- Educate patients on their medical conditions and prescribed medications
- Identify gaps in care and issue recommendations to close them
- No prescribing or creation of treatment plans required
Qualifications:
- Valid Nurse Practitioner license in Missouri
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months of experience as a nurse practitioner preferred
