Deliver high-impact care in the homes of Medicare Advantage plan members throughout Wake County, NC. This per diem opportunity is ideal for a Nurse Practitioner seeking flexibility, meaningful patient interaction, and autonomy. Your advanced assessment skills will be essential for delivering comprehensive in-home risk evaluations, helping improve outcomes for individuals in Raleigh, Cary, Apex, and the surrounding areas.

Schedule:

• Flexible shifts offered between Monday and Sunday
• Build your own schedule to suit your availability
• Minimum hourly commitment per week or bi-weekly, set to your preference

Key Responsibilities:

• Conduct annual and post-discharge risk assessments in home settings for Medicare Advantage members
• Review comprehensive medical histories, current conditions, and medication use
• Educate members on their chronic conditions and prescribed medications
• Detect gaps in patient care and share actionable recommendations for improvement
• Focus exclusively on assessmentsno prescribing medications or developing treatment plans is required

Qualifications:

• Active Nurse Practitioner License in North Carolina
• Completion of an MSN from an accredited school of nursing
• AANP or ANCC board certification
• Current BLS/CPR certification
• Minimum of 6 months recent experience as a Nurse Practitioner preferred
