Take on a vital role in advancing patient well-being by performing home health risk assessments for Medicare Advantage plan members across Luzerne County, PAincluding Wilkes-Barre, Hazleton, and Nanticoke. This per diem opportunity offers flexibility and the chance to deliver essential care directly to patients in their own homes.

Schedule:

• Flexible shifts available Monday through Sunday
• Ability to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete comprehensive annual risk assessments in patient homes, focusing on evaluating current and prior medical history, conditions, and medication use
• Provide thorough education to patients about their health conditions and medications
• Detect gaps in care and suggest actionable recommendations for clinical teams
• Visits will not require you to develop treatment plans or prescribe medications
• Assessments are generally 45 minutes in duration

Qualifications:

• Active and unrestricted Nurse Practitioner license in Pennsylvania
• Board Certification from AANP or ANCC
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• Minimum of six months recent experience as a Nurse Practitioner is preferred
