Step into a valued role performing comprehensive home health risk assessments for Medicare Advantage plan members across Boone County, MO and neighboring communities. This per diem position offers the flexibility to tailor your own scheduleshifts are available between Monday and Sunday, giving you full control over your work-life balance. Youll have the opportunity to visit patients in their homes throughout Columbia, Rocheport, McBaine, Ashland, and additional areas within the county, conducting evaluations that play a crucial part in their ongoing care.
Schedule:
- Choose flexible shifts from Monday through Sunday
- Set your own schedule
- Commit to a minimum weekly or bi-weekly hours as suits your availability
Key Responsibilities:
- Conduct annual or post-discharge home health risk assessments, averaging 45 minutes per visit
- Review and evaluate patients medical histories, current medical conditions, and medication records
- Provide patient education regarding diagnoses and medications
- Identify care gaps and make recommendations to address them
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in Missouri
- MSN degree from an accredited nursing program
- Current BLS (CPR) certification
- Board Certification (AANP or ANCC)
- At least 6 months of recent NP experience preferred
