Deliver expert patient care and make a difference by performing comprehensive home health risk assessments. As a per diem Nurse Practitioner specializing in home health, youll conduct visits with Medicare Advantage members throughout Jefferson County, MO. Assessments typically take about 45 minutes and may involve annual wellness checks or post-discharge follow-ups. Enjoy the autonomy of creating your own schedule with flexibility across locations such as Arnold, Imperial, Festus, and Barnhart.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Self-scheduled work to suit your availability
- Set your own minimum weekly or bi-weekly hours
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members in patients homes
- Review and evaluate patients medical history, current health status, and medication lists
- Provide patient education regarding medical conditions and prescribed medications
- Identify care gaps and offer recommendations to address them directly to care teams
- No prescription writing or treatment planning requiredfocus on assessment and education
Qualifications:
- Active Nurse Practitioner license in Missouri
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- Current CPR certification (BLS)
- A minimum of 6 months of recent nurse practitioner experience is preferred
Compensation: Earn $80$90 per evaluation, in addition to a mileage stipend and paid administrative time.
