Advance your Nurse Practitioner career by providing essential home health risk assessments for Medicare Advantage members throughout Boone County, MO. This per diem opportunity is ideal for practitioners seeking autonomy and a flexible schedule while making a meaningful impact in the community. Youll visit patients in their homeslocations may include Columbia, Rocheport, McBaine, Ashland, and other areas within the countyconducting comprehensive health assessments that typically last about 45 minutes.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own schedule to fit your availability
- Commit to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Perform thorough annual health risk assessments and post-discharge evaluations for Medicare Advantage plan members
- Review prior and current medical history, assess ongoing conditions, and document medication usage
- Deliver patient education regarding medical conditions and prescriptions
- Identify gaps in care and make recommendations to improve health outcomes
- No requirements to prescribe medications or create treatment plans
Qualifications:
- Active Nurse Practitioner License in Missouri
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months Nurse Practitioner experience is preferred
- MSN degree from an accredited nursing program
