Shape patient outcomes and support community health by performing in-depth home health risk assessments for Medicare Advantage members. As a Nurse Practitioner specializing in home health, you'll enjoy a per diem opportunity with flexible scheduling throughout Marion County, Illinoisincluding Centralia, Salem, and Sandoval. Design your workweek around your life with the ability to choose your shifts and define your own weekly or bi-weekly commitment.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Create your own schedule based on your availability
- Minimum weekly or bi-weekly hourly commitment to suit your needs
Key Responsibilities:
- Complete comprehensive annual risk assessments in home settings for Medicare Advantage members
- Review and evaluate each members past and present medical history, ongoing medical concerns, and medication usage
- Provide education regarding chronic health conditions and medications for patients in their homes
- Recognize and outline recommendations to address any identified gaps in care
- Visits may include annual health assessments or post-discharge follow-ups
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license for Illinois
- Current CPR certification (BLS required)
- Board certification through AANP or ANCC
- Minimum of 6 months recent nurse practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
