Job Description
Make an impact in patient care by performing in-home health risk assessments for Medicare Advantage members across Suffolk County, Massachusetts. This per diem opportunity is ideal for Nurse Practitioners with experience or interest in home health who value flexibility and autonomy. You will conduct comprehensive clinical evaluations, supporting preventative care for patients residing in Boston, Revere, Chelsea, and surrounding areas.
Schedule:
- Flexible shifts available between Monday and Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members in their homes
- Review and evaluate each patients prior and current medical history, ongoing conditions, and medication regimen
- Educate patients on medical conditions, medication usage, and promote improved health practices
- Identify gaps in care and offer actionable recommendations to close those gaps
- No requirement for prescribing medications or creating treatment plans
Qualifications:
- Active Nurse Practitioner license in Massachusetts
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- At least 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited nursing school
This role offers pay ranging from $80 to $90 per evaluation, along with a mileage stipend and compensation for administrative duties.
