Are you passionate about delivering personalized care to patients in their homes? This per diem opportunity allows Nurse Practitioners to conduct meaningful home health risk assessments for Medicare Advantage plan members across various locations in Guernsey County. The role offers exceptional flexibility and independence, with practitioners conducting focused, patient-centered visits averaging 45 minutes each. Your expertise will help ensure high-quality, individualized care and support for communities such as Cambridge Township, Cambridge, and Jackson Township.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingset your own work hours
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive home health risk assessments
- Review and evaluate prior/current medical history, conditions, and medication use
- Educate patients on their medical conditions and prescribed medications
- Identify and address gaps in care, offering recommendations for improved outcomes
- Visits may include annual health assessments or post-discharge evaluations
- No requirement to create treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in OH
- Board Certification (AANP or ANCC)
- Current BLS/CPR certification
- Master of Science in Nursing from an accredited institution
- Minimum of 6 months experience as a Nurse Practitioner preferred
