Schedule:

• Flexible shifts available any day, Monday through Sunday

• Commit to a minimum number of weekly or bi-weekly hours based on your availability

Key Responsibilities:

• Conduct home health risk assessments, each typically lasting 45 minutes, with visits ranging from annual assessments to post-discharge follow-ups
• Review patients prior and current medical history, conditions, and medication use
• Educate patients about their health conditions and prescribed medications
• Identify and address any gaps in care, offering actionable recommendations
• No requirement to create treatment plans or prescribe medications

Preferred Qualifications:

• Active Nurse Practitioner license in Tennessee
• Current BLS/CPR certification
• Board Certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• At least six months of current nurse practitioner experience preferred
