Deliver meaningful care with a flexible per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. In this role, you'll visit Medicare Advantage plan members throughout Middlesex Countyincluding Cambridge, Lowell, and Newtonto conduct assessments that support preventive care and ongoing wellness. Each appointment typically lasts around 45 minutes, ranging from annual evaluations to post-discharge visits.

Schedule:

• Flexible shifts available seven days a week
• Design your own work schedule
• Minimum commitment required on a weekly or bi-weekly basis

Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members
• Review and document prior and current medical histories, health conditions, and medications
• Educate patients on chronic conditions and prescribed treatments
• Detect and address gaps in care, making recommendations for follow-up as needed
• Enjoy a focused clinical scope: no treatment plans or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in Massachusetts
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• MSN from an accredited nursing program
• At least 6 months of recent NP experience preferred
