Healthcare professionals with a passion for community-based care are invited to take on a dynamic per diem nurse practitioner role in home health within Hutchinson County, TX. In this role, you will conduct comprehensive risk assessments for Medicare Advantage plan members in their residences across Borger, Stinnett, Fritch, and other nearby locales.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduleddesign shifts that fit your availability
- Minimum weekly or bi-weekly hourly commitment required
Primary Responsibilities:
- Complete annual and post-discharge home health risk assessments, typically lasting around 45 minutes per visit
- Review and evaluate each patients medical history, current conditions, and medication usage
- Offer education to patients regarding their medical states and prescribed medications
- Identify gaps in care and provide professional recommendations to optimize health outcomes
- No need to create treatment plans or prescribe medicationfocus is on assessment and guidance
Qualifications:
- Unrestricted Nurse Practitioner license, valid in Texas
- Active CPR certification (BLS required)
- Board certification via AANP or ANCC
- At least 6 months of nurse practitioner experience preferred
- MSN degree from an accredited nursing institution
