Deliver impactful care directly in patients homes with this per diem opportunity for an experienced Nurse Practitioner specializing in home health. This position focuses on conducting essential home health risk assessments for Medicare Advantage members within Union County, including Blairsville, Suches, and Lake Nottely. Your expertise will play a vital role in helping improve patient outcomes while you enjoy autonomy and flexibility in your workday.

Schedule:

• Flexible shifts offered Monday through Sunday
• Self-scheduling: create your own work timetable
• Choose a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage plan members
• Review and evaluate patients medical histories, current conditions, and medications during each visit
• Deliver education to patients regarding their health conditions and medications
• Identify care gaps and offer recommendations for follow-up or coordination of care
• Visits are focused on assessments only; no requirements for prescribing or treatment planning

Qualifications:

• Valid Nurse Practitioner license in Georgia
• Current CPR (BLS) certification
• Board certified through the AANP or ANCC
• At least 6 months of recent experience as a Nurse Practitioner preferred
• MSN degree from an accredited nursing program
