Deliver expert care as a skilled Nurse Practitioner providing home health risk assessments for Medicare Advantage plan members in Haywood County, TN. This per diem opportunity allows you to impact patient well-being while enjoying remarkable scheduling flexibility and autonomy. Assignments will take place throughout the area, including Brownsville, Stanton, and other neighboring communities.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Choose your preferred weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive, 45-minute home health risk assessments for Medicare Advantage members
• Review and evaluate medical history, current medical conditions, and medication records
• Educate patients regarding diagnoses, medications, and self-care practices
• Identify care gaps and provide actionable recommendations to improve patient outcomes
• Perform visits for annual health assessments or post-discharge follow-ups
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Current Nurse Practitioner license in Tennessee
• Active CPR (BLS) certification
• Board Certification via AANP or ANCC
• At least 6 months of recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
