Make a meaningful impact in patient care by providing individualized home health risk assessments as a Nurse Practitioner. In this flexible per diem opportunity, you will visit Medicare Advantage plan members for scheduled assessments, supporting quality healthcare across New Bern, Havelock, Vanceboro, Bridgeton, and other communities throughout Craven County.

Schedule:

• Flexible shifts available Monday through Sunday
• Customize your own work schedule
• Commit to weekly or bi-weekly minimum hours

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage members, with each visit typically lasting 45 minutes
• Review and evaluate prior and current medical history, active conditions, and medication profiles
• Deliver patient education regarding medical conditions, medications, and overall health maintenance
• Identify health or care gaps, and offer recommendations to help close those gaps
• Focus on assessmentsno treatment planning or medication prescribing involved

Qualifications:

• Active Nurse Practitioner (NP) license in North Carolina
• Current BLS (CPR) certification
• Board certification from AANP or ANCC
• At least 6 months of recent experience as a Nurse Practitioner preferred
• Master of Science in Nursing (MSN) from an accredited institution
