A per diem opportunity is available for a Nurse Practitioner specializing in home health assessments. This role involves performing in-home risk assessments for Medicare Advantage plan members in Barry County, MO, including areas such as Monett, Savannah, Country Club, and Fillmore. The focus is on annual health assessments and post-discharge visits, with each visit averaging around 45 minutes.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Set your own schedule according to your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual risk assessments for individuals enrolled in a Medicare Advantage health plan
- Review patients previous and current medical histories, chronic conditions, and medications
- Provide educational information regarding health conditions and medication usage
- Identify care gaps and make appropriate recommendations to address them
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Missouri
- Current CPR (BLS) certification
- Board Certification as an NP (AANP or ANCC)
- Master of Science in Nursing (MSN) degree from an accredited institution
- At least six months of current experience as a Nurse Practitioner preferred
Compensation: Earn between $80 and $90 per evaluation, with additional stipends for mileage and administrative responsibilities.
