Schedule/Hours:
- Contract assignment; schedule discussed during interview.
Required Qualifications:
- Active LPN license in good standing
- At least one year of recent home health experience
- Proficiency with HomeCare HomeBase (HCHB) EMR
- Strong clinical assessment and documentation skills
- Reliable transportation for regular home visits
- Ability to work independently while coordinating across care teams
Key Responsibilities:
- Deliver skilled nursing care in patient homes, following individualized care plans
- Monitor and document each patients progress, updating care plans as needed
- Administer medications and carry out treatments per state and regulatory guidelines
- Educate patients and families about disease prevention and ongoing care management
- Maintain accurate records using the HomeCare HomeBase (HCHB) EMR platform
- Collaborate effectively with RNs, therapists, and other healthcare professionals to ensure holistic patient care
