Provide compassionate, patient-centered nursing care as a Licensed Practical Nurse (LPN) in a home health setting. This role emphasizes promoting patient independence, education, and recovery while collaborating with an interdisciplinary healthcare team. The position involves managing a caseload of patients within a designated territory, ensuring all care and documentation meet regulatory standards. Experience with Home Care Home Base (HCHB) and home health compliance is essential.

Schedule:

• Monday through Friday
• 8:00 AM 5:00 PM (with occasional schedule flexibility to meet patient needs)

Responsibilities:

• Conduct skilled nursing visits to assess, plan, and provide individualized care in patients homes
• Administer medications, perform wound care, manage IV therapy, and deliver clinical interventions as ordered
• Monitor patient conditions and communicate changes promptly to RN Case Managers or physicians
• Educate patients and caregivers on disease management, medications, and self-care techniques
• Document all assessments and care using Home Care Home Base (HCHB) software accurately
• Collaborate with the care team to ensure compliance with the plan of care
• Uphold infection control and patient safety standards at all times
• Participate in case conferences, quality improvement activities, and required trainings

Qualifications:

• Active, unrestricted LPN license in the state of practice
• Minimum one year of experience in home health, hospice, or community-based care
• Proficiency with Home Care Home Base (HCHB) system
• Valid drivers license, reliable insured vehicle, and reliable transportation
• Current Basic Life Support (BLS) certification
• Strong clinical judgment, organizational skills, and ability to work independently

Preferred Experience:

• Previous experience in Medicare-certified home health agencies
• Thorough knowledge of home health compliance and documentation standards
• Excellent communication and a patient-focused approach
