An excellent opportunity awaits a Licensed Practical Nurse (LPN) with expertise in home health care. This contract assignment focuses on providing skilled, compassionate care for geriatric patients in a home setting, utilizing your clinical skills and home health experience.
Schedule/Hours:
- Contract assignment (with potential for extension)
Key Qualifications:
- Valid and active LPN license
- At least one year of recent home health experience
- Proficiency using HomeCare HomeBase (HCHB) electronic medical records
- Strong ability in clinical assessments and detailed documentation
- Reliable transportation to conduct home visits efficiently
- Self-motivated, able to manage a patient caseload independently
Responsibilities:
- Deliver high-quality nursing care to patients in their homes following individualized care plans
- Monitor patient conditions, update care plans, and document progress accurately in HCHB EMR
- Administer medications and perform nursing treatments in accordance with state guidelines
- Educate patients and caregivers on disease prevention and management strategies
- Collaborate with registered nurses, therapists, and other healthcare professionals for optimal patient outcomes
