Deliver exceptional care and make a difference in the lives of geriatric patients as a Licensed Practical Nurse specializing in home health services. This contract role provides the opportunity to work one-on-one with patients in their homes, ensuring their comfort, safety, and wellbeing while using your skills to guide and educate both patients and their families.

Required Qualifications:
- Active and unrestricted LPN license
- Minimum one year of recent experience in a home health setting
- Proficiency with HomeCare HomeBase (HCHB) EMR system
- Demonstrated clinical assessment abilities and excellent documentation skills
- Dependable transportation for daily home visits
- Capability to work independently and manage a dynamic caseload
- Commitment to high-quality care and excellent patient outcomes
Primary Responsibilities:
- Deliver skilled nursing care based on established care plans within patients homes
- Keep detailed and accurate records using HomeCare HomeBase (HCHB) EMR
- Monitor health status and adjust care plans as necessary
- Administer prescribed medications and treatments in accordance with state regulations
- Provide health education and support to patients and their families on disease management, medication protocols, and preventive measures
- Collaborate with a multidisciplinary healthcare team to coordinate comprehensive care
