Job Description
Provide skilled, compassionate nursing care as a Home Health Licensed Practical Nurse (LPN) serving patients in their homes within the Navarre, FL area. This full-time contract position emphasizes supporting patient independence, recovery, and education while maintaining strong communication with patients, families, and the healthcare team.
Responsibilities include conducting skilled nursing visits to assess, plan, and implement individualized care plans. You will administer medications, perform wound care, manage IV therapy as needed, and monitor changes in patient conditions, reporting promptly to the RN Case Manager or physician. Educating patients and caregivers on disease management and self-care techniques is essential. Accurate documentation using Home Care Home Base (HCHB) is required, along with collaborating with the interdisciplinary team to ensure compliance and quality care. Maintaining infection control standards and participating in quality improvement activities are key components of the role.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM with occasional flexibility for patient needs
- Approximately 35 patient visits per day (2532 points weekly)
Qualifications:
- Active, unrestricted LPN license in Florida
- Minimum 1 year of home health, hospice, or community-based nursing experience
- Proficiency with Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable insured vehicle for travel within assigned territory
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently in a field-based setting managing a dynamic caseload
Preferred:
- Experience with Medicare-certified home health agencies
- Expertise in home health compliance and documentation standards
- Excellent interpersonal skills with a patient-centered approach
