A rewarding opportunity is available for a skilled and compassionate Registered Nurse to deliver high-quality care in a home health setting. This role involves providing direct nursing care to patients in their homes, ensuring personalized treatment plans, and collaborating with interdisciplinary teams to foster optimal patient outcomes. Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, assess changes in condition, and promptly coordinate with physicians and care teams.
• Educate patients and families about disease processes, treatment plans, medications, and preventive care.
• Maintain accurate documentation using Home Care Home Base (HCHB) and adhere to regulatory standards.
• Collaborate with therapists, social workers, and home health aides to ensure coordinated care.
• Participate in case conferences, staff meetings, and continuing education as required.

Qualifications:

• Active RN license in good standing.
• Minimum of one year recent experience in Home Health nursing with field experience preferred.
• Proficiency in Home Care Home Base (HCHB) and OASIS assessments required.
• Strong clinical assessment and critical thinking skills.
• Reliable transportation with a valid driver's license and insurance.
• Excellent communication, time management, and organizational abilities.
• BSN preferred.
• Experience managing 2532 points per week and knowledge of Medicare/home health compliance standards are advantageous.

Schedule & Location:

• Full-time, 13-week contract.
• Monday through Friday, 8:00 AM 5:00 PM.
• Caseload of approximately 35 patient visits daily.
• Work is field-based with daily travel to patient homes within the assigned territory; mileage reimbursement provided.
• Located in Marietta, GAa vibrant community known for its rich history, welcoming neighborhoods, and accessible amenities.
