Key Qualifications:
- Active and unrestricted LPN license
- At least one year of recent experience in a home health setting
- Proficiency with HomeCare HomeBase (HCHB) EMR system
- Strong clinical skills in patient assessment, documentation, and care planning
- Reliable transportation for travel to patient residences
- Exceptional ability to manage a caseload and work autonomously
Primary Responsibilities:
- Deliver skilled nursing care within patient homes, following individualized care plans
- Monitor patient progress, administer medications, and perform treatments under state nursing guidelines
- Maintain thorough, timely documentation in the HCHB EMR platform
- Educate patients and families on ongoing care needs, disease prevention, and effective health management
- Collaborate with registered nurses, therapists, and allied health professionals to ensure continuity of care and coordinated patient support
