Schedule/Hours:

Key Qualifications:

  • Active and unrestricted LPN license
  • At least one year of recent experience in a home health setting
  • Proficiency with HomeCare HomeBase (HCHB) EMR system
  • Strong clinical skills in patient assessment, documentation, and care planning
  • Reliable transportation for travel to patient residences
  • Exceptional ability to manage a caseload and work autonomously

Primary Responsibilities:

  • Deliver skilled nursing care within patient homes, following individualized care plans
  • Monitor patient progress, administer medications, and perform treatments under state nursing guidelines
  • Maintain thorough, timely documentation in the HCHB EMR platform
  • Educate patients and families on ongoing care needs, disease prevention, and effective health management
  • Collaborate with registered nurses, therapists, and allied health professionals to ensure continuity of care and coordinated patient support
