An opportunity is available for an experienced Licensed Practical Nurse (LPN) to provide compassionate home health care to geriatric patients. This contract position focuses on delivering high-quality healthcare in patients homes and requires a nurse who thrives in an independent, field-based role and is comfortable using electronic medical records.
Schedule/Hours:
- Contract assignment, with potential for extension
Key Qualifications:
- Valid and active LPN license
- At least one year of recent experience in home health care
- Proficient in using HomeCare HomeBase (HCHB) EMR
- Demonstrated competency in nursing assessment and clinical documentation
- Reliable transportation is essential for daily home visits
- Ability to manage varying caseloads and work independently
Responsibilities:
- Deliver skilled nursing care in accordance with individualized care plans
- Monitor patient progress and document care consistently in HCHB EMR
- Administer prescribed medications and perform treatments per state guidelines
- Educate patients and families regarding health management and preventative care
- Maintain meticulous patient records and ensure accurate, up-to-date documentation
- Collaborate effectively with registered nurses, therapists, and other healthcare professionals to coordinate holistic patient care
