An opportunity is available for a Licensed Practical Nurse (LPN) with home health experience to provide patient-centered nursing care in a home setting. This role involves delivering compassionate care while promoting patient independence and recovery. The position is contract-based, full-time, and field-based within the assigned territory around Laurens, SC.

Schedule/Hours:
- Monday Friday, 8:00 AM 5:00 PM (with occasional flexibility based on patient needs)
Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care plans in patients homes
- Administer medications, perform wound care, and manage clinical interventions such as IV therapy
- Monitor and report changes in patient conditions to RN Case Managers or physicians
- Educate patients and caregivers about disease management, medications, and self-care techniques
- Document all assessments and care in Home Care Home Base (HCHB) accurately
- Collaborate with the interdisciplinary healthcare team to coordinate care and ensure compliance
- Maintain infection control and patient safety standards
- Participate in case conferences, quality improvement initiatives, and training sessions
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of experience in home health, hospice, or community-based care
- Proficiency in Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable vehicle, and current insurance
- Current BLS certification
- Strong clinical judgment, time management, and communication skills
- Ability to work independently and manage a dynamic caseload
Preferred Experience:
- Experience with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation standards
- Excellent interpersonal skills with a patient-focused care approach
