An opportunity is available for a Licensed Practical Nurse (LPN) with home health experience to provide patient-centered nursing care in a home setting. This role involves delivering compassionate care while promoting patient independence and recovery. The position is contract-based, full-time, and field-based within the assigned territory around Laurens, SC.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM (with occasional flexibility based on patient needs)

Responsibilities:

• Conduct skilled nursing visits to assess, plan, and implement individualized care plans in patients homes
• Administer medications, perform wound care, and manage clinical interventions such as IV therapy
• Monitor and report changes in patient conditions to RN Case Managers or physicians
• Educate patients and caregivers about disease management, medications, and self-care techniques
• Document all assessments and care in Home Care Home Base (HCHB) accurately
• Collaborate with the interdisciplinary healthcare team to coordinate care and ensure compliance
• Maintain infection control and patient safety standards
• Participate in case conferences, quality improvement initiatives, and training sessions

Qualifications:

• Active, unrestricted LPN license in the state of practice
• Minimum 1 year of experience in home health, hospice, or community-based care
• Proficiency in Home Care Home Base (HCHB) charting system
• Valid drivers license, reliable vehicle, and current insurance
• Current BLS certification
• Strong clinical judgment, time management, and communication skills
• Ability to work independently and manage a dynamic caseload

Preferred Experience:

• Experience with Medicare-certified home health agencies
• In-depth knowledge of home health compliance and documentation standards
• Excellent interpersonal skills with a patient-focused care approach
