Deliver compassionate nursing care directly to patients where they are most comfortabletheir homes. As a Home Health Licensed Practical Nurse (LPN), youll make a meaningful impact in patient lives by promoting independence, providing education, and guiding recovery. This full-time contract role combines clinical expertise with autonomy, requiring a dedicated professional who values both high standards and empathetic care.
Schedule and Work Setting:
- Monday Friday, 8:00 AM 5:00 PM (with occasional flexibility for patient needs)
- Field-based, serving patients in their homes within a local territory
- Weekly caseload equivalent to approximately 35 patient visits daily (2532 points per week)
- Requires reliable transportation
Key Responsibilities:
- Conduct skilled nursing home visits, assess patient health status, and implement tailored care plans
- Administer medications, manage IV therapies, and perform wound care as prescribed
- Closely monitor changes in patient conditions, reporting promptly to the RN Case Manager or physician
- Educate patients and caregivers on disease management and self-care strategies
- Maintain accurate documentation using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary teams, participate in case conferences, and uphold infection control standards
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum of 1 years experience in home health, hospice, or similar setting
- Proficiency with Home Care Home Base (HCHB) system
- Valid drivers license with reliable transportation and current insurance
- BLS certification
- Strong clinical, communication, and organizational skills
- Ability to work independently, managing a dynamic field caseload
Preferred Experience:
- Background with Medicare-certified home health agencies
- Deep understanding of compliance and documentation standards
- Patient-focused approach with excellent interpersonal abilities
