This contract position focuses on supporting geriatric patients with skilled nursing services, requiring experience in home health and proficiency in HomeCare HomeBase (HCHB) EMR.
Schedule/Hours:
- Contract position with flexible scheduling to meet patient needs
Key Responsibilities:
- Deliver direct skilled nursing services in patient residences as outlined in individualized care plans
- Monitor patient progress, adjust care plans, and ensure accurate documentation
- Perform medication administration and treatments according to state regulations
- Provide education for patients and families on care management and disease prevention
- Maintain up-to-date records utilizing HomeCare HomeBase (HCHB) EMR
- Collaborate with RNs, therapists, and other healthcare providers for holistic patient care
Preferred Qualifications:
- Active, unrestricted LPN license
- At least one year of recent experience working in a home health setting
- Demonstrated ability with HomeCare HomeBase (HCHB) electronic medical records
- Excellent clinical assessment and documentation skills
- Self-motivated with strong organizational skills to manage a patient caseload
- Reliable transportation for travel to patient homes
