Providing compassionate and expert care in patients homes, this opportunity offers a rewarding role for a Licensed Practical Nurse (LPN) experienced in home health nursing. Job Type: Full-time Contract
Schedule:
- Monday Friday
- 8:00 AM 5:00 PM (with occasional flexibility for patient needs)
Key Responsibilities:
- Conduct comprehensive skilled nursing visits including assessments, care planning, and implementation tailored to individual home health patients
- Administer medications, wound care, and manage clinical interventions such as IV therapy as prescribed
- Monitor and report changes in patient conditions promptly to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medication administration, and self-care techniques
- Maintain accurate and thorough documentation using Home Care Home Base (HCHB) software
- Coordinate patient care in collaboration with the interdisciplinary team and adhere to compliance standards
- Uphold infection control practices and ensure safety during home visits
- Participate in case conferences and quality improvement initiatives
Qualifications and Desired Experience:
- Active and unrestricted LPN license in the state of practice
- Minimum one year of experience in home health, hospice, or community-based nursing
- Proficient in Home Care Home Base (HCHB) documentation system
- Valid driver's license, reliable vehicle, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication skills, and ability to operate independently in the field
Preferred:
- Experience with Medicare-certified home health agencies
- Solid understanding of home health compliance and documentation requirements
- Excellent interpersonal skills with a compassionate, patient-centered approach
