Key Responsibilities:
- Provide skilled nursing care in accordance with individualized patient care plans
- Monitor and document patient status, updating care documentation as needed
- Administer medication and perform treatments under state regulations
- Educate patients and their families on management of care and disease prevention
- Accurately maintain records using HomeCare HomeBase (HCHB) EMR
- Collaborate with registered nurses, therapists, and other healthcare professionals for comprehensive care
Schedule/Hours:
- Contract assignment with potential for extension
Desired Qualifications:
- Active LPN license
- At least one year of recent home health experience required
- Proficient in using HomeCare HomeBase (HCHB) EMR
- Reliable transportation to conduct home visits
- Strong assessment, documentation, and communication skills
- Ability to work independently and efficiently manage a caseload
