Qualifications:
- Active LPN license in good standing
- At least one year of recent experience in home health
- Proficiency using HomeCare HomeBase (HCHB) electronic medical records
- Outstanding clinical assessment and documentation skills
- Reliable transportation for home visits
- Ability to manage a caseload independently and efficiently
Key Responsibilities:
- Deliver skilled nursing care to patients in their homes as outlined in individualized care plans
- Monitor and document each patients progress, making updates to care plans when necessary
- Administer medications and perform treatments in accordance with state and regulatory guidelines
- Provide patient and family education on disease management and preventive care
- Maintain meticulous records utilizing the HomeCare HomeBase (HCHB) EMR system
- Collaborate closely with registered nurses, therapists, and other health professionals to support comprehensive care
