Job Description

Deliver high-quality care to geriatric patients as a Licensed Practical Nurse focused on home health services. This contract position offers the chance to make a meaningful impact by supporting patients in the comfort of their own homes while utilizing your clinical expertise and proficiency with home health documentation systems.

Schedule:

  • Contract assignment (duration discussed upon interview)

Qualifications:

  • Active LPN license required
  • At least one year of recent home health experience
  • Proficient with HomeCare HomeBase (HCHB) EMR for documentation
  • Strong clinical assessment abilities and meticulous documentation skills
  • Dependable transportation to conduct home visits
  • Demonstrated ability to work independently and manage a home health caseload

Key Responsibilities:

  • Provide skilled nursing care based on individualized care plans
  • Monitor patients status, documenting progress and updating care plans accordingly
  • Administer prescribed medications and perform indicated treatments within state guidelines
  • Educate patients and family members on ongoing care and strategies for disease prevention
  • Maintain accurate, timely records using HomeCare HomeBase (HCHB) EMR
  • Collaborate effectively with RNs, therapists, and interdisciplinary teams to ensure comprehensive patient care
