Deliver high-quality care to geriatric patients as a Licensed Practical Nurse focused on home health services. This contract position offers the chance to make a meaningful impact by supporting patients in the comfort of their own homes while utilizing your clinical expertise and proficiency with home health documentation systems.
Schedule:
- Contract assignment (duration discussed upon interview)
Qualifications:
- Active LPN license required
- At least one year of recent home health experience
- Proficient with HomeCare HomeBase (HCHB) EMR for documentation
- Strong clinical assessment abilities and meticulous documentation skills
- Dependable transportation to conduct home visits
- Demonstrated ability to work independently and manage a home health caseload
Key Responsibilities:
- Provide skilled nursing care based on individualized care plans
- Monitor patients status, documenting progress and updating care plans accordingly
- Administer prescribed medications and perform indicated treatments within state guidelines
- Educate patients and family members on ongoing care and strategies for disease prevention
- Maintain accurate, timely records using HomeCare HomeBase (HCHB) EMR
- Collaborate effectively with RNs, therapists, and interdisciplinary teams to ensure comprehensive patient care
