Providing compassionate, high-quality nursing care in patients homes, this role is designed for an experienced Home Health Licensed Practical Nurse (LPN) who values patient independence and education. The position involves managing a caseload of homebound patients within an assigned territory around Fountain Inn, SC, focusing on skilled assessments, care planning, and ongoing communication with interdisciplinary healthcare teams.

Responsibilities:

• Perform skilled nursing visits to assess patient needs and implement individualized care plans.
• Administer medications, provide wound care, and manage IV therapy or other clinical interventions as prescribed.
• Monitor patient conditions and promptly report significant changes to RN Case Managers or physicians.
• Educate patients and caregivers on disease processes, medication routines, and self-care techniques.
• Document all patient assessments and care accurately using Home Care Home Base (HCHB).
• Collaborate with the healthcare team to coordinate care and maintain plan compliance.
• Uphold infection control standards and ensure patient safety at all times.
• Participate in case conferences, quality improvement efforts, and ongoing training.

Qualifications:

• Active, unrestricted LPN license for the state.
• Minimum one year of experience in home health, hospice, or community-based care.
• Proficient with Home Care Home Base (HCHB) charting system.
• Current Basic Life Support (BLS) certification.
• Valid drivers license, reliable vehicle, and insurance.
• Strong clinical judgment, time management, and communication skills.
• Ability to work independently managing a dynamic caseload.

Preferred:

• Experience in Medicare-certified home health agencies.
• Knowledge of home health compliance and documentation standards.
• Patient-centered mindset with excellent interpersonal skills.

Schedule & Workload:

• Full-time, contract position.
• Monday through Friday, 8:00 AM to 5:00 PM, with occasional flexibility for patient needs.
• Approximately 35 patient visits daily (2532 points weekly).
