Desired Qualifications:
- Current, active LPN license
- At least one year of recent home health experience
- Strong working knowledge of HomeCare HomeBase (HCHB) EMR
- Excellent clinical assessment and documentation abilities
- Reliable personal transportation for patient home visits
- Independent work ethic with strong caseload management skills
Core Responsibilities:
- Deliver skilled nursing care in accordance with individualized care plans
- Monitor patient progress, administer medications, and update care plans as needed
- Perform treatments under state regulations and best practices
- Educate patients and their families on care management and preventive health
- Accurately chart and maintain patient records using HCHB EMR
- Collaborate with RNs, therapists, and healthcare professionals to ensure continuity of care
