A rewarding opportunity is available for an experienced Home Health Licensed Practical Nurse (LPN) to deliver exceptional nursing care in patients homes in the Crestview, FL area. This field-based, full-time contract position focuses on promoting patient independence, education, and recovery while ensuring compliance with home health policies and procedures.
Responsibilities include conducting skilled nursing visits to assess and implement individualized care plans, administering medications and treatments such as wound care and IV therapy, monitoring patient conditions, and collaborating closely with the interdisciplinary healthcare team. Accurate documentation using Home Care Home Base (HCHB) is essential, as is patient and caregiver education. You will manage a caseload of approximately 35 visits per day, working Monday through Friday from 8:00 AM to 5:00 PM, with occasional flexibility to meet patient needs.
Qualifications:
- Active and unrestricted LPN license in Florida
- Minimum 1 year of home health, hospice, or community-based nursing experience
- Proficiency with Home Care Home Base (HCHB) electronic charting
- Valid drivers license, reliable vehicle, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently managing a dynamic caseload
Preferred:
- Experience with Medicare-certified home health agencies
- Solid understanding of home health compliance and documentation standards
