Job Description

A rewarding opportunity is available for a skilled and compassionate Licensed Practical Nurse to provide essential home health care services. This field-based position focuses on delivering patient-centered support while empowering clients to achieve optimal independence and well-being. Successful candidates will utilize strong clinical skills, prioritize patient safety, and collaborate closely with families and interdisciplinary care teams.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM
  • Occasional flexibility may be needed for patient care needs

Qualifications:

  • Active and unrestricted LPN license in the state of practice
  • Minimum of 1 year of recent experience in home health, hospice, or similar community-based setting
  • Proficiency in Home Care Home Base (HCHB) required
  • Current BLS (Basic Life Support) certification
  • Valid drivers license, reliable personal vehicle, and current auto insurance
  • Strong clinical judgment and communication skills
  • Ability to effectively manage a dynamic caseload and work independently

Preferred Qualifications:

  • Experience with Medicare-certified home health agencies
  • Familiarity with home health compliance, documentation standards, and regulations
  • Excellent interpersonal skills and a patient-centric approach

Key Responsibilities:

  • Manage a caseload of 2532 points weekly, typically 35 patient visits per day
  • Conduct comprehensive assessments, plan and implement individualized care plans
  • Deliver skilled nursing services, including medication administration, wound care, and IV therapy
  • Document all interventions and communications in Home Care Home Base (HCHB)
  • Educate patients and families on disease processes, medications, and self-management strategies
  • Collaborate with healthcare providers to ensure continuity of care
  • Maintain infection control and safety standards at all times
  • Participate in case conferences and ongoing education
