An opportunity is available for a skilled Home Health Registered Nurse (RN) to deliver compassionate, patient-centered care in home settings. This role requires providing clinical nursing services with a focus on comprehensive assessment, care planning, and coordination within an interdisciplinary team. The ideal candidate thrives in a self-directed environment, maintains excellent communication, and upholds high standards of nursing excellence.
Key Responsibilities:
- Conduct thorough OASIS assessments and develop personalized care plans.
- Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify condition changes, and communicate updates promptly to physicians and care teams.
- Educate patients and families on health conditions, treatments, medications, and preventive care.
- Document accurately and timely within the Home Care Home Base (HCHB) electronic system.
- Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides.
- Participate in case conferences, team meetings, and continuing education.
- Comply with all regulatory and agency standards governing patient care and documentation.
Qualifications and Desired Experience:
- Active RN license in good standing for the state.
- At least 1 year of recent home health nursing experience, preferably field-based.
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools is required.
- Strong clinical assessment, critical thinking, and time management skills.
- Reliable transportation, valid drivers license, and insurance for travel between patient homes.
- Ability to work independently with a flexible, field-based schedule.
- BSN preferred.
- Experience managing caseloads of 2532 points weekly and knowledge of Medicare and home health regulations is a plus.

Schedule and Details:
- Full-time contract, 13-week duration
- Monday through Friday, 8:00 AM 5:00 PM
- Caseload averaging 25-32 points per week (3-5 patient visits daily)
- Daily travel within assigned territory with mileage reimbursement
