Job Description

Requirements:

RN Licensure
Minimum of one year of recent home health experience
Active BLS Certification (American Heart Association)
Proficiency in HCHB, Oasis, and Outlook
Experience with a home-to-home workload

Responsibilities:

Collaborate with healthcare professionals to develop home care treatment plans
Record and track patient conditions and progress
Administer medication and provide basic wound care
Manage multiple points per day in a home-to-home setting

Details:

Home-to-home responsibilities
Full-time position: 40 hours per week (8:00 AM to 5:00 PM)
Mileage reimbursement provided
