Job Description

Sunbelt Staffing is seeking a compassionate and dedicated Registered Nurse (RN) to join a Home Health team in the picturesque community of Hermitage, Pennsylvania.

As a Home Health RN, you will have the opportunity to make a significant impact on the lives of patients by providing high-quality care in the comfort of their homes.

Job Details:

Position Type: Contract, Full-Time
Length: 13 Week Contract, possibility of extension upon successful completion and performance
Weekly Hours: 40 hours per week, Monday to Friday, 8:00 AM - 5:00 PM Additional Availability: Occasional weekends and on-call
Location: Hermitage, PA
Benefits: Medical, Dental, Vision, 401k
Compensation: Hourly pay, not by visit; Negotiable pay, based off territory and experience; Blended pay packages available for travelers; Mileage reimbursement

Job Duties:

Perform comprehensive assessments of patients' health conditions and home care needs, tailoring care plans to meet each patient's specific goals and preferences.
Collaborate closely with patients, their families, and the healthcare team to develop individualized care plans focused on maximizing patient independence and well-being.
Administer medications, provide wound care, and conduct necessary medical procedures with a high level of precision and adherence to best practices.
Monitor and maintain meticulous electronic records of patients' vital signs, symptoms, and responses to treatment.
Educate patients and their families on disease management, treatment plans, and preventive measures, emphasizing patient empowerment and self-management.
Facilitate effective communication and coordination among interdisciplinary healthcare professionals, ensuring comprehensive and seamless patient care.

Requirements:

Active Pennsylvania RN license
Active AHA or American Red Cross BLS certification
Minimum of 1 year of Home Health experience
Proficiency in Oasis and Homecare Homebase software to be considered for hire
Reliable transportation
Ability to pass a drug screen and background check
