Job Description

Are you a dedicated and experienced Registered Nurse seeking a rewarding position in home health care? We have an exciting opportunity for a Home Health RN in Buffalo. Join our dynamic team and provide high-quality care to patients in the comfort of their own homes.

Responsibilities:

Deliver comprehensive nursing care to patients in a home health setting
Conduct patient assessments and develop individualized care plans
Administer medications and treatments as prescribed
Monitor and document patient progress
Collaborate with interdisciplinary teams to ensure coordinated care
Educate patients and families on self-care techniques and health management
Maintain accurate and up-to-date patient records

Qualifications:

Valid and current Registered Nurse (RN) license
Minimum of 6 months of home health experience
Proficiency in using Oasis documentation
Experience with HCHB electronic medical record system preferred
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Valid driver's license and reliable transportation
