Location: Wheeling, WV

Job Type: Full-Time, Contract

Work Schedule: Monday to Friday, 8:00 am to 5:00 pm (occasional weekends and on-call)

Hours: 40 hours per week

Compensation: Hourly rate, negotiable based on candidate's experience; Blended pay packages available for travelers

Benefits: Medical benefits, dental, vision, 401k, mileage reimbursement

Responsibilities:

Provide high-quality nursing care to patients in their homes
Assess patient needs and develop individualized care plans
Administer medications and treatments as prescribed by healthcare professionals
Monitor and document patient progress and update care plans as needed
Collaborate with a multidisciplinary team to ensure holistic patient care
Educate patients and their families on disease management and self-care techniques

Requirements:

Active LPN license in the state of West Virginia
Minimum of 1 year of home health experience
Proficiency in using Homecare Homebase software
Strong clinical skills and ability to work independently
Excellent communication and interpersonal skills
Valid driver's license and reliable transportation
Ability to travel to patients' homes within designated geographic areas

Due to the nature of home health care, occasional overtime may be required, but it is not common. Overtime will be compensated accordingly.
