Home Health LPN Opportunity in Pittsfield, MA

We are seeking a dedicated Licensed Practical Nurse (LPN) for a fulfilling home health position in Pittsfield, MA! This role is perfect for an experienced LPN with a background in home health and familiarity with HomeCare HomeBase or managing a home-to-home patient schedule.

Position Highlights:

Personalized Patient Care: Provide direct patient support, including medication administration, wound care, and comprehensive health assessments within patients' homes.
Autonomous Scheduling: Efficiently manage a home-to-home schedule to ensure high-quality care delivery.
Detailed Record-Keeping: Accurately document patient information using HomeCare HomeBase, supporting care continuity and team communication.

Requirements:

Active LPN licensure in Massachusetts.
At least one year of recent experience in home health (private duty experience not applicable).
Proficiency with HomeCare HomeBase or experience in a home-to-home patient care setting.
Current BLS certification from the American Heart Association.

Position Details:

Schedule: Full-time, 40 hours per week, Monday through Friday, 8:00 AM - 5:00 PM.
Mileage Reimbursement: Provided for travel within Pittsfield and neighboring areas.
