Job Title: Home Health Licensed Practical Nurse (LPN)
Location: Lawrence, MA

Duration: 13 weeks

Shift: Monday-Friday, with potential on-call

Job Description:
Sunbelt Staffing is seeking a dedicated Home Health Licensed Practical Nurse (LPN) for an assignment in Lawrence, MA. This role involves providing high-quality nursing care to patients in their homes, managing a varied caseload, and ensuring accurate documentation using Homecare Homebase. Key Responsibilities:

Deliver skilled nursing care in accordance with individualized home health care plans.
Administer medications, perform wound care, and educate patients and families on health management.
Monitor patient progress, updating care plans as needed.
Document all patient care accurately and timely using Homecare Homebase.
Collaborate with healthcare professionals to provide comprehensive patient care.

Qualifications:

Active LPN license in Massachusetts or compact state licensure.
Minimum of 3 years of LPN experience, with at least 1 year of recent home health care experience.
Proficiency with Homecare Homebase is required.
Current BLS certification.
Reliable transportation and a valid driver’s license.
Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission.
