Job Description:
Sunbelt Staffing is seeking a dedicated Home Health Licensed Practical Nurse (LPN) for a 13-week contract in Shelton, CT. This position is perfect for an LPN with a passion for providing quality in-home care and a strong background in home health nursing.

Requirements:

Minimum of 1 year of home health experience
Proficiency in Home Care Home Base charting system
Active LPN license in the state of Connecticut

Responsibilities:

Provide compassionate, patient-centered care in a home health setting
Conduct assessments, administer medications, and provide wound care
Collaborate with a multidisciplinary team to develop and update patient care plans

Schedule:

13-week contract, with potential for extension based on performance and need
