Job Description

There is a Full-Time, Home Health opportunity available in San Antonio, Texas for a Licensed Practical Nurse.

The hired Licensed Practical Nurse (LPN) should have at least six months of Home Health experience and be proficient in Homecare Homebase.

Job Duties:

Provide skilled nursing care and treatment to patients in their home.
Use a comprehnsive care plan and provide solutions.
Spend quality time with patient in their home, to promote patients health and independence.
Assess, monitor, and document progress of patients' health.
Other duties as assigned.

Job Details:

13-Week Contract
40 Hours per week
Monday-Friday, 8am-5pm
Occasional Weekends &On-Call
Mileage reimbursement
Medical benefits, dental, vision, 401k

Job Requirements:

Active TX LPN License
Active AHA BLS
At least 1 year of LPN experience
At least 6-Months of HH, LPN experience
Must have Homecare Homebase experience
Joint Commission Onboarding upon offer
