Job Description

Description: Looking for a skilled and compassionate Registered Nurse (RN) to join our team on a 13-week contract basis in Fort Smith, AR. As a Home Health RN, you will play a vital role in providing high-quality care to patients in their homes. If you're passionate about home health and have the required qualifications, we want to hear from you!

Responsibilities:

  1. Perform comprehensive assessments of patients' health status and develop personalized care plans.
  2. Administer medications, perform wound care, and provide other skilled nursing interventions as needed.
  3. Conduct regular home visits to monitor patients' progress, assess their needs, and provide education and support.
  4. Collaborate with the interdisciplinary team to coordinate care and ensure continuity of services.
  5. Document all patient interactions, assessments, and interventions accurately and in accordance with agency policies and regulatory standards.
  6. Communicate effectively with patients, families, and caregivers to promote understanding of care plans and promote patient independence.
  7. Adhere to all infection control protocols and safety measures to ensure patient and staff well-being.

Requirements:

  • Active RN license in the state of Arkansas.
  • Current Basic Life Support (BLS) certification from the American Heart Association or the American Red Cross.
  • Minimum of 1 year of experience as a Home Health RN.
  • Proficiency in using electronic medical records systems, including HomeCare/HomeBase and Oasis.
