Job Description

Sunbelt Staffing, a premier staffing agency, is seeking a dedicated and experienced Registered Nurse (RN) for a contract position with a leading health insurance provider. This role is based in Providence, RI, and involves responsibilities within the Institutional Special Needs Plan (ISNP) program, focusing on providing comprehensive care for insurance members in nursing homes and assisted living facilities. The primary goal of the ISNP program is to treat patients in place when appropriate to avoid unnecessary hospitalizations and improve overall healthcare outcomes.

Schedule: Full-Time, Monday - Friday, 8:00 AM - 5:00 PM
Hours:40 hours per week
Dress Code: Business Casual

ISNP Program Duties:

Complete routine monthly visits as delegated by the NP.
Review residents' status and assist in achieving progress toward their healthcare goals.
Attend meetings on behalf of the NP and communicate updates to the Interdisciplinary Care Team (ICT).
Perform care coordination activities within the SNF to ensure residents' needs are met.
Assist with other clinical and quality responsibilities for residents, such as identifying gaps in care.
Communicate and engage with residents, families, SNF staff, and other members of the ICT.
Regularly communicate with the Advanced Practice Clinician (APC) regarding resident status and facility needs.
Perform resident rounds and assist with Advance Care Planning (ACP).
Support the NP in managing the overall care plan by identifying gaps in care and providing recommendations to the patient’s primary care provider.
Prescribe new or refill non-narcotic medications upon consultation with the patient’s primary care provider if needed.

Requirements:

Valid Registered Nurse (RN) license in the state of Rhode Island.
Experience in primary/preventive care and care coordination.
Ability to identify acute illness triggers and manage chronic conditions.
Strong interpersonal and communication skills, with the ability to work collaboratively with healthcare professionals and patients' families.
Commitment to improving the overall quality of care and reducing avoidable hospitalizations.
COVID Vaccination required per client.
