Experience with OASIS is required for this assignment.

Responsibilities:

  • Conduct comprehensive patient assessments to evaluate their health status, including physical, emotional, and social factors.
  • Develop and implement individualized care plans based on assessment findings, physician orders, and patient goals.
  • Administer skilled nursing care, including wound care, medication administration, IV therapy, and other nursing procedures.
  • Educate patients and their families on disease management, medication regimens, and self-care techniques.
  • Monitor and document patient progress, making necessary adjustments to the care plan as needed.
  • Collaborate with interdisciplinary healthcare teams, including physicians, therapists, and social workers, to ensure holistic patient care.
  • Maintain accurate and timely electronic medical records in compliance with regulatory guidelines.

Requirements:

  • Current and unrestricted Registered Nurse (RN) license
  • Minimum of 6 months of geriatric home health experience
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills to effectively interact with patients and their families.
  • Compassion and a patient-centered approach to care.
  • Valid driver's license and access to reliable transportation.
