Our client is a prominent nationwide home health agency committed to delivering high-quality care to patients in their homes. Position Details:

Position: Home Health LPN
Location: Crestview Hills, KY
Contract Length: 13 weeks
Employment Type: Full-Time, 40 hours per week
Schedule: Monday to Friday, 8 AM - 5 PM, with one weekend per month on-call
Productivity: 25-30 points of productivity per week
Mileage Reimbursement: Provided from patient to patient

Job Duties:

Provide skilled nursing care to patients in their homes according to the plan of care
Administer medications, treatments, and procedures as prescribed
Monitor and document patient conditions and progress
Educate patients and their families on health care needs and instructions
Collaborate with the interdisciplinary team to ensure high-quality patient care
Participate in on-call rotation as scheduled

Requirements:

Active LPN license in the state of Kentucky
Active AHA or American Red Cross BLS certification
Minimum of one year of home health experience with an agency
Proficiency in Homecare Homebase electronic medical records system
Must have reliable transportation
Joint Commission Onboarding standards upon offer
